MAKING AN APPOINTMENT

DD FORM 1172-2 OPTIONS (without being present with dependents)
-Sponsor can sign it in an ID Card Facility and have it put in the system
-Sponsor can print form out, fill it out, and get it notarized

**Sponsor is more than welcome to come in with dependents to fill out the form**
SPONSOR UNWILLING TO SIGN THE DD FORM 1172-2
If the Sponsor is unwilling to sign the DD Form 1172-2 for a dependent, the dependent can come in the ID Card facility and receive a 30 day temporary card which gives us time to mail out the needed forms to contact the Sponsor and for them to respond. After the 30 days is up and the Sponsor has not provided documentation as to why the dependent cannot receive an ID card, the dependent will then receive the normal dependent ID card. Two valid forms of unexpired ID are required for all dependents 18 and older. Note: Need the Sponsor's SSN or DOD ID number to start the process.
PROOF OF IDENTITY IS MANDATORY FOR ALL TYPES OF MILITARY ID CARDS
Two (2) valid, unexpired forms of identification must be presented to receive any ID card if 18 or older. Refer to the DOD List of Acceptable Documents. All documents must be originals or they must be certified true copies.
POWER OF ATTORNEY
If your Military Sponsor is unable to accompany you to your visit, a valid "General" or "Special" Power of Attorney will allow you to conduct DEERS/ID Card business on your sponsor's behalf.
LOST/STOLEN ID CARDS (ALL CAC ID HOLDERS)
Military Members: A counseling statement is required and signed by your unit Chain of Command. For Civilian Personnel/ Contractors: a memo from their Supervisor. A police report will also suffice as long as CAC is listed among the things stolen.
LOST/STOLEN CARDS (Retirees, Dependents, DAVs)
Retirees: Must have 2 forms of unexpired ID
Dependents: Must have 2 forms of unexpired ID, sponsor must be present and or a DD1172-2/Special/General Power of Attorney. Anyone over age 18 must have two forms of ID for re-issue. Copy of police report, if reported.
DAVs: Must have 2 forms of unexpired ID
DRESS AND APPEARANCE FOR MILITARY PERSONNEL
Active Duty, Reserve, National Guard will need to be within the "Regulation Standards" for ID issue, (Clean shaven, hair, make-up, jewelry). It is recommended to wear light colored clothing for the photo to allow birth date to be visible on the CAC.
CAC RESET: INVALID PIN/BLOCKED/LOCKED ONLY
No appointment is needed for a CAC pin reset ONLY if you received an error stating you have been "BLOCKED" or "LOCKED" or you have an "INVALID PIN".
ELIGIBILITY REQUIREMENTS, FAMILY MEMBERS
Must have 2 valid, unexpired forms of identification and one of the following: Sponsor present, unexpired Power of Attorney, original DD Form 1172 (ID Card Application) signed by the Sponsor and a Verifying Official (uploaded in DEERS) or signed by the sponsor and Notarized.
ADDING FAMILY MEMBERS, CHILDREN
Birth certificate and social security card (if received). Hospital verification of facts will suffice to add a newborn. (If parents are not married, see illegitimate child section below)
ADDING FAMILY MEMBERS, ILLEGITIMATE CHILD
Birth certificate, social security card, and one of the following if the sponsor is male: Approved Dependency Determination from DFAS, court order establishing paternity, or Affidavit of Parentage for Child Born out of Wedlock.
ADDING FAMILY MEMBERS, STEP CHILDREN
Marriage Certificate/License, birth certificate and social security card
ADDING FAMILY MEMBERS, ADOPTED CHILDREN
Final Adoption Decree or documentation from authorized placement agency under the Department of Human Services, birth certificate and social security card.
ADDING FAMILY MEMBERS, WARD
Court order granting custody to the Sponsor. The court order must stipulate that the child will be in the care and custody of the Sponsor for no less than twelve (12) consecutive months, Birth certificate and SS card.
ADDING FAMILY MEMBERS, CHILD BORN OVERSEAS
FS 240 (Consular Report of Birth Abroad) or DS Form 1350 (Certification of Report of Birth) and social security card.
ADDING FAMILY MEMBERS, SPOUSE
Marriage Certificate/License (no gold seal), birth certificate, SS Card, photo ID issued by state or local government. If your spouse is a foreign national, original marriage certificate, valid passport, and birth certificate will need to include a certified translation into English.
Active Service Member: Name change transaction with unit has to be done to update DEERS; DAV/Retiree/Dependent Spouse: Marriage Cert/License, Divorce Decree, Court Order, or State certification; Dependent Child: New birth cert and Court order/state certification.
ELIGIBILITY REQUIREMENTS, FULL TIME STUDENTS
21-23: Must be attending an accredited institution, (2) valid, unexpired forms of identification and letter from Student Registrar's Office or National Clearing House stating they are enrolled full time in an accredited college in pursuit of an Associate's Degree or higher, current dates of enrollment and expected date of graduation.
ELIGIBILITY REQUIREMENTS, 100% DAV
Letter from Department of Veteran Affairs showing member as a honorably discharged veteran rated as 100% service connected, authorizing commissary store and exchange privileges, DD Form 214 (Copy 4), SS Card, valid photo ID.
DFAS PACKETS, INCAPCITATED CHILD (ask ID Card Facility for packet)
DFAS approval (DD 1172 and DD 27-88), photo ID, SS card and medical sufficiency statement (memorandum from doctor).
DFAS PACKETS, PARENT/PARENT IN LAW (ask ID Card Facility for packet)
DFAS approval (DD 1172 and DD 27-88), photo ID, SS card, birth certificate and birth certificate of sponsor. Parent-in-law: in addition to above documentation please include birth certificate and marriage certificate of sponsor and spouse.
DFAS PACKETS, WARD (ask ID Card Facility for packet)
DFAS approval (DD 1172 and DD 27-88), birth certificate, SS card and photo ID if over 18 yrs of age.
ELIGIBILITY REQUIREMENTS, RETIREES
Initial Retirement- Active Duty: Retirement Orders and DD Form 214 (Copy 4). Reserve/National Guard Retirees: Orders/Letter transferring to Retired List and authorizing retired pay or Retired Reserve. If Medicare eligible, must present Medicare Card.
Age 60 Retirement- retirement packet must be submitted by the retiree to HRC in order to receive the retirement orders from them.
ELIGIBILITY REQUIREMENTS, DOD CONTRACTORS
All data must be previously input and verified through the Contractor Verification System (CVS). If going overseas, a Letter of Authorization (LOA) is required. Must have two (2) valid, unexpired forms of identification.
ELIGIBILITY REQUIREMENTS, MILITARY CAC
All data must be previously inputted and all transactions must be complete. Must have two (2) valid, unexpired forms of identification.
Customers will need to know their appointment date and time to cancel/reschedule an appointment. If more than 15 minutes late for your appointment, you will need to reschedule or may be seen as a walk-in.
If you are turning age 65 and will be eligible for TRICARE FOR LIFE, bring your Medicare card to your ID appointment, along with 2 forms of ID. One of the IDs must be a picture ID, and must not be expired. Sponsor required or valid Power of Attorney presented.
If student falls below full-time status, the ID card must be turned in to local DEERS office. Only newborns and foreign nationals can be enrolled without a SS Card. The SS Card must be provided once received to continue receiving medical benefits. All foreign documents must have an Apostille and English translation. Please contact the Department of Foreign Ministry Affairs in the country of issuance or google "Apostille certification & country" to obtain guidance.

ON ACTIVE DUTY ORDERS
Army National Guard ADOS/AGR: Make sure the orders are in iPERMS and send to J1 Automation (firstname.lastname@example.org) for the Sidpers transactions to be done.
Other National Guard and Reserve branches: As an Army National Guard facility, we cannot upload your orders or complete your transactions. When you arrive at our office, the orders have to already be reflected in the DEERS system. If you are having issues, the contact numbers are below:
