MAKING AN ONLINE APPOINTMENT

 

We are currently working hard to make this available to our customers.  Please check back frequently for the online appointment scheduler that will be located here on our home page.

 

SPONSOR UNABLE TO ATTEND APPOINTMENT

 

If your sponsor is unable to attend the appointment, please have them follow the link and update/replace the ID Card of the dependent in need, which will let them complete the required DD1172-2.

TIPS FOR A SUCCESSFUL VISIT

 

Bring ALL required documents-see specific requirements, any questions, please call (984)664-6250 before appointment. All documents must be in English or include a certified translation. Note a Birth Registration Card

does not take the place of a Birth Certificate.

 

REQUIRED DOCUMENTS for ID Cards

 

ALL Age 18 and older must have TWO forms of ID to get a new ID card. One of them must be an unexpired state or government issued photo ID. EX:Passport, Driver License, SSAN Card.

Under 18 requires one form of ID. EX:Birth Certificate.

 

POWER OF ATTORNEY

 

If your Military Sponsor is unable to accompany you to your visit, a valid "General" or "Special" Power of Attorney will allow you to conduct DEERS/ID business on your sponsor's behalf. You may also provide the original signed & verified DD Form 1172-2 da.

 

LOST/STOLEN ID CARDS (ALL CAC ID HOLDERS)

 

A counseling statement is required and signed by your unit Chain of Command.  For Civilian Personnel/ Contractors: a statement from their Supervisor. A police report will also suffice as long as CAC is listed among the things

stolen.

 

LOST/STOLEN CARDS for Dependents

 

Dependents: Must have at least one federal or state issued photo I.D., sponsor must be present and or a DD1172-2/Special/General Power of Attorney.

Anyone over age 18 must have two forms of ID for re-issue. Copy of police report, if reported.

 

DRESS AND APPEARANCE FOR MILITARY PERSONNEL

 

Active Duty, Reserve, National Guard.will need to be within the "Regulation Standards" for ID issue. (Clean shaven, hair, make-up, jewelry). It is recommended to wear light colored clothing for the photo to allow birth date to be visible on the CAC.

 

ADDING A SPOUSE TO DEERS

 

The sponsor needs to bring the following for their spouse: Marriage Certificate, Birth certificate, SS card, & a photo ID. All must be originals or certified copies. If your spouse is a foreign national- original marriage certificate, valid passport, birth certificate will need to include a certified translation into English.

 

DEPENDENT ID CARDS

 

For dependent ID cards, the sponsor must be present to sign the DD Form 1172. If not available, the dependent must have a previously signed 1172 (within 90 days of signature) signed by their sponsor or valid Power of Attorney.

 

CHILDREN BORN OUT OF WEDLOCK

 

Female Sponsors: Birth Certificate and social security card; Male sponsors: Court order establishing paternity, an approved Dependency Determination letter from DFAS or a State VAP (Voluntary Acknowledgement of Paternity)

Form, Birth Certificate, SSN Card.

 

CHILDREN OVER 21 AND FULL TIME STUDENT STATUS

 

All full-time College students MUST bring a letter from their school registrar's office stating they are enrolled full time in an accredited college in pursuit of an Associate Degree or higher, dates of enrollment and expected date of graduation. Sponsor will also need to certify on the DD

Form 1172 (ID Card Application) that he/she is providing over 50% support.  Benefits may then be extended to age 23 or date of graduation; whichever comes first.

 

PARENT ENROLLMENT

 

You will need to provide a DFAS letter of approval, Service Members Birth Certificate & a valid Photo ID. A photo ID, Birth certificate and SSAN Card of the parent will need to be provided as well. Parent In-Law, You will need to provide a DFAS letter of approval**, Service Members SPOUSES Birth Certificate. A photo ID, Birth certificate and SSAN Card of the parent-in-law will need to be provided as well. **Must reapply prior to ID renewal.

 

ENROLLING DEPENDENT WARD

 

Requires a DFAS letter of approval, Court Document placing the child in the member's household for at least 12 consecutive months, Birth Cert and SSAN Card. All documents must be translated into English and certified. **Must reapply prior to ID renewal.

 

INCAPACITATED CHILD OVER 21

 

You will need to provide a DFAS letter of approval**, Medical Sufficiency Statement, a photo ID and a second form of identification (Birth Certificate, Social Security Card, passport, student ID) **Must reapply prior to ID renewal.

 

WALK-IN CUSTOMERS

 

All walk-in customers are seen on a first come, first serve basis. Wait times for walk-ins can exceed 3+ hours depending upon the number of customers waiting for service. Appointments have priority. If you do not have an appointment, we cannot guarantee that you will be seen that day.

 

DISABLED AMERICAN VETERAN RATED AT 100%

 

You will need a commissary letter from the VA, rating you at 100% disabled due to service connected disability (Award/Rating Package not accepted), DD Form 214, Birth Certificate, SSN Card and photo ID. See adding spouse or

child for additional requirements.

 

CANCEL/RESCHEDULE APPOINTMENTS

 

Customers will need to know their appointment date and time to cancel/reschedule an appointment. If more than 15 minutes late for your appointment, you will need to reschedule or may be seen as a walk-in.

 

TURNING 65?

 

If you are turning age 65 and will be eligible for TRICARE FOR LIFE, bring your Medicare card to your ID appointment, along with 2 forms of ID. One of the IDs must be a picture ID, and must not be expired. Sponsor required or valid POA presented.

 Acceptable Forms of Identification

Primary Identity Source Document
  • U.S. Passport or a U.S. Passport Card;
  • Permanent resident Card or an Alien Registration Receipt Card (Form I-551);
  • Foreign passport;
  • Employment Authorization Document that contains a photograph (Form I-766);
  • Driver's license or an identification (ID) card issued by a state or possession of the United States provided it contains a photograph;
  • U.S. Military ID card; or
  • U.S. Military dependent's ID card; or
  • Personal Identity Verification (PIV) Card

Secondary Identity Source Document

The secondary identity source document may be from the list above, but cannot be of the same type as the primary identity source document.1

  • U.S. Social Security Card issued by the Social Security Administration
  • Original or certified copy of a birth certificate issued by a state, county, municipal authority, possession, or outlying possession of the United States bearing an official seal
  • ID card issued by a federal, state, or local government agency or entity, provided it contains a photograph
  • Voter's registration card
  • U.S. Coast Guard Merchant Mariner Card
  • Certificate of U.S. Citizenship (Form N-560 or N-561)
  • Certificate of Naturalization (Form N-550 or N-570)
