MAKING AN APPOINTMENT

Please visit https://rapids-appointments.dmdc.osd.mil

                                       OR

Call the ID Card Office at 984-664-6250 (leave your name and number and we will call you back within 2 business days)

 

DD FORM 1172-2 OPTIONS (without being present with dependents)

-Sponsor can sign it in an ID Card Facility and have it put in the system

-Sponsor can print form out, fill it out, and get it notarized

-Sponsor can fill out the form online (must be a replacement card, not initial) https://www.dmdc.osd.mil/self_service

**Sponsor is more than welcome to come in with dependents to fill out the form**

 

PROOF OF IDENTITY IS MANDATORY FOR ALL TYPES OF MILITARY ID CARDS

Two (2) valid, unexpired forms of identification must be presented to receive any ID card if 18 or older.  Refer to the I-9 List of Acceptable Documents.  All documents must be originals or they must be certified true copies.

 

POWER OF ATTORNEY 

If your Military Sponsor is unable to accompany you to your visit, a valid "General" or "Special" Power of Attorney will allow you to conduct DEERS/ID business on your sponsor's behalf.

 

LOST/STOLEN ID CARDS (ALL CAC ID HOLDERS)

A counseling statement is required and signed by your unit Chain of Command.  For Civilian Personnel/ Contractors: a statement from their Supervisor.  A police report will also suffice as long as CAC is listed among the things stolen.

 

LOST/STOLEN CARDS (Retirees, Dependents, DAVs)

Retirees:  Must have 2 forms of unexpired ID

Dependents: Must have 2 forms of unexpired ID, sponsor must be present and or a DD1172-2/Special/General Power of Attorney.  Anyone over age 18 must have two forms of ID for re-issue. Copy of police report, if reported.

DAVs:  Must have 2 forms of unexpired ID

 

DRESS AND APPEARANCE FOR MILITARY PERSONNEL

Active Duty, Reserve, National Guard will need to be within the "Regulation Standards" for ID issue, (Clean shaven, hair, make-up, jewelry).  It is recommended to wear light colored clothing for the photo to allow birth date to be visible on the CAC.

 

CAC RESET:  INVALID PIN/BLOCKED/LOCKED ONLY

No appointment is needed for a CAC reset ONLY if you received an error stating you have been "BLOCKED" or "LOCKED" or you have an "INVALID PIN".

 

ELIGIBILITY REQUIREMENTS, FAMILY MEMBERS

Must have 2 valid, unexpired forms of identification and one of the following: Sponsor present, unexpired Power of Attorney, original DD Form 1172 (ID Card Application) signed by the Sponsor and a Verifying Official or signed by the sponsor and Notarized.

 

ADDING FAMILY MEMBERS, CHILDREN

(If parents are not married, see illegitimate child section below), birth certificate and social security card (if received). Hospital verification of facts will suffice to add a newborn.

 

ADDING FAMILY MEMBERS, ILLEGITIMATE CHILD

Birth certificate, social security card, and one of the following if the sponsor is male: Approved Dependency Determination from DFAS, court order establishing paternity, or Affidavit of Parentage for Child Born out of Wedlock.

 

ADDING FAMILY MEMBERS, STEP CHILDREN

Marriage Certificate/License, birth certificate and social security card

 

ADDING FAMILY MEMBERS, ADOPTED CHILDREN

Final Adoption Decree or documentation from authorized placement agency under the Department of Human Services, birth certificate and social security card.

 

ADDING FAMILY MEMBERS, WARD

Court order granting custody to the Sponsor. The court order must stipulate that the child will be in the care and custody of the Sponsor for no less than twelve (12) consecutive months, Birth certificate and SS card.

 

ADDING FAMILY MEMBERS, CHILD BORN OVERSEAS

FS 240 (Consular Report of Birth Abroad) or DS Form 1350 (Certification of Report of Birth) and social security card.

 

ADDING FAMILY MEMBERS, SPOUSE

Marriage Certificate/License, birth certificate, SS Card, photo ID issued by state or local government. If your spouse is a foreign national, original marriage certificate, valid passport, and birth certificate will need to include a certified translation into English.

 

NAME CHANGE FOR SPOUSE UPON ENROLLMENT

The name may only be changed if the spouse provides 2 identity source documents such as a Social Security Card and valid Driver's License/Passport reflecting the name change.

 

ELIGIBILITY REQUIREMENTS, FULL TIME STUDENTS

21-23: Must be attending an accredited institution, Sponsor must be providing over 50% support, (2) valid, unexpired forms of identification and letter from Student Registrar's Office or National Clearing House stating they are enrolled full time in an accredited college in pursuit of an Associate's Degree or higher, dates of enrollment and expected date of graduation.

 

ELIGIBILITY REQUIREMENTS, 100% DAV

Letter from Department of Veteran Affairs showing member as a honorably discharged veteran rated as 100% service connected, authorizing commissary store and exchange privileges, DD Form 214 (Copy 4), SS Card, valid photo ID.

 

DFAS PACKETS, INCAPCITATED CHILD

DFAS approval (DD 1172 and DD 27-88), photo ID, SS card and medical sufficiency statement (memorandum from doctor).

 

DFAS PACKETS, PARENT/PARENT IN LAW

DFAS approval (DD 1172 and DD 27-88), photo ID, SS card, birth certificate and birth certificate of sponsor. Parent-in-law: in addition to above documentation please include birth certificate and marriage certificate of sponsor and spouse.

 

DFAS PACKETS, WARD

DFAS approval (DD 1172 and DD 27-88), birth certificate, SS card and photo ID if over 18 yrs of age.

 

ELIGIBILITY REQUIREMENTS, RETIREES

Initial Retirement: Retirement Orders and DD Form 214 (Copy 4). Reserve/National Guard Retirees: Orders/Letter transferring to Retired List and authorizing retired pay. If Medicare eligible, must present Medicare Card.

 

ELIGIBILITY REQUIREMENTS, DOD CONTRACTORS

All data must be previously input and verified through the Contractor Verification System (CVS). If going overseas, a Letter of Authorization (LOA) is required. Must have two (2) valid, unexpired forms of identification.

 

ELIGIBILITY REQUIREMENTS, MILITARY CAC

All data must be previously inputted and all transactions must be complete.  Must have two (2) valid, unexpired forms of identification.

 

CANCEL/RESCHEDULE APPOINTMENTS

Customers will need to know their appointment date and time to cancel/reschedule an appointment. If more than 15 minutes late for your appointment, you will need to reschedule or may be seen as a walk-in.

 

TURNING 65?

If you are turning age 65 and will be eligible for TRICARE FOR LIFE, bring your Medicare card to your ID appointment, along with 2 forms of ID. One of the IDs must be a picture ID, and must not be expired. Sponsor required or valid Power of Attorney presented.

 

NOTES

If student falls below full-time status, the ID card must be turned in to local DEERS office. Only newborns and foreign nationals can be enrolled without a SS Card. The SS Card must be provided once received to continue receiving medical benefits. All foreign documents must have an Apostille and English translation. Please contact the Department of Foreign Ministry Affairs in the country of issuance or google "Apostille certification & country" to obtain guidance.

 Acceptable Forms of Identification

Primary Identity Source Document
  • U.S. Passport or a U.S. Passport Card;
  • Permanent resident Card or an Alien Registration Receipt Card (Form I-551);
  • Foreign passport;
  • Employment Authorization Document that contains a photograph (Form I-766);
  • Driver's license or an identification (ID) card issued by a state or possession of the United States provided it contains a photograph;
  • U.S. Military ID card; or
  • U.S. Military dependent's ID card; or
  • Personal Identity Verification (PIV) Card

Secondary Identity Source Document

The secondary identity source document may be from the list above, but cannot be of the same type as the primary identity source document.1

  • U.S. Social Security Card issued by the Social Security Administration
  • Original or certified copy of a birth certificate issued by a state, county, municipal authority, possession, or outlying possession of the United States bearing an official seal
  • ID card issued by a federal, state, or local government agency or entity, provided it contains a photograph
  • Voter's registration card
  • U.S. Coast Guard Merchant Mariner Card
  • Certificate of U.S. Citizenship (Form N-560 or N-561)
  • Certificate of Naturalization (Form N-550 or N-570)
  • U.S. Citizen ID Card (Form I-197)
  • Identification card for use of resident Citizen in the United States (Form I-179),
  • Certification of Birth Abroad or Certification of Report of Birth Issued by the Department of State (form FS-545 or Form DS-1350);
  • Temporary resident card (Form I-688);
  • Employment Authorization card (Form I-688A);
  • Reentry Permit (Form I-327);
  • Refugee Travel Document (Form I-571)
  • Employment authorization document issued by Department of Homeland Security (DHS)
  • Employment Authorization Document issued by DHS (Form I-688B);
  • Foreign ID with photograph;
  • Driver's license issued by a Canadian government entity; or
  • Native American tribal document.

For example, if the primary source document is a foreign passport (e.g., Italy), the secondary source document should not be another foreign passport (e.g., France).
